SERVING THE ENTIRE BAY AREA

The goods and services that are shown on this price list are those we can provide to our families.

You may choose only the items you desire. HOWEVER, ANY FUNERAL ARRANGEMENTS YOU SELECT WILL INCLUDE A CHARGE FOR OUR BASIC SERVICE AND OVERHEAD. If legal or other requirements mean you must purchase any item you did not specifically ask for, we will explain the reason in writing on statement we provide describing the funeral goods and service you selected.

Prior to drafting any contract for goods or services, the responsible party or the decedent's survivor. Who is handling the funeral arrangements is entitled to receive a copy of any pre-need agreement in the possession of the funeral establishment that has been signed and paid for, in full or in part, by or on behalf of the decedent.

This list does not include prices for any Cash Advance items that you may ask us to purchase for you, such as an organist and / or other musicians, flowers, clergy honoraria, obituary notice, certified copies of the Death Certificate, Burial Permit, Cemetery or Crematory advances, or other gratuities. The prices for those items will be shown on your bill or the statement describing the funeral goods and services you selected.

You may request this mortuary to perform certain services that we do not have on this General Price List. Charges for those certain services will be provided in writing before such services are rendered.

PAYMENT POLICY

Payments for the arrangements you have selected is due prior to service time.

The following methods of payment are available for your convenience: cash, check, or major credit card. We will accept an assignable insurance policy upon verification. As dedicated professional we are committed to serving your family. Should you have difficulty meeting these obligations please discuss your situation, in confidence, with your funeral director.

DISCLAIMER OF WARRANTY

The only warranty of the casket or any other merchandise sold in connection with service is the express written warranty, if any, granted by the manufacture. This Funeral Home makes no warranty, express or implied, including an implied warranty or merchantability, and an implied warranty of fitness for a particular purpose, with respect to the casket and/or outer burial container.***The mortuary takes no responsibility for the strength, integrity or appearance of containers furnished by purchaser. Containers must be of a material and design acceptable by the crematory or cemetery. THERE IS NO SCIENTIFIC OR OTHER EVIDENCE THAT ANY CASKET WITH A SEALING DEVICE WILL PRESERVE HUMAN REMAINS.

PROFESSIONAL SERVICES FEES

BASIC PROFESSIONAL SERVICES OF FUNERAL DIRECTOR, STAFF AND OVERHEAD $ 555.00

This fee is for the basic services of funeral director and staff and will be added to the total cost of the funeral arrangements you select and is non-declinable. This fee is already included in our charges for DIRECT CREMATION, IMMEDIATE BURIAL, and FORWARDING OR RECEIVING REMAINS.

This fee includes:

· Personnel available 24 hours a day, 365 days a year to respond to initial call.
· The arrangement conference.
· Coordinating funeral ceremony/service plains with clergy, cemetery, and/or other individuals involved in the final disposition of the deceased.
· Clerical assistance in the completion of various forms associated with the funeral.
· Obtaining all legal documents, certificates and permits.

TRANSFER OF DECEASED $ 195.00

· Additional charge per man for Transfer. $ 100.00

EMBALMING AND PREPARATION ROOM $ 485.00

Embalming is a chemical process which provides temporary preservation of the body and eliminates certain health hazards. A person licensed by the state of California must complete this procedure. Except in certain cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have a right to choose an arrangement that does not require you to pay for it, such as a direct cremation or immediate burial.

TRI-CITY CREMATION & FUNERAL SERVICE REQUIRES EMBALMING OF THE REMAINS IF A FAMILY CHOOSES TO HOLD AN “OPEN CASKET” VISITATION AND/OR AN “OPEN CASKET” SERVICE.

OTHER PREPARATION OF THE BODY

1. Bathing, Dressing, Casketing, Cosmetology and Hair of embalmed remains $ 195.00
2. Sanitary Care, Dressing, Casketing of non-embalmed remains $ 195.00
3. Autopsy or tissue donor care $ 225.00
4. Identification of the Deceased (Viewing) Family Only – No Embalming $ 325.00

Use of room up to one hour for immediate family only
5. Refrigeration, in compliance with California State Law to require refrigeration
Of an non-embalmed remains. (After first 48 hours $75.00 per day) $75.00

USE OF FACILITES AND STAFF FOR FUNERAL/MEMORIAL CEREMONY

FEES ARE AS FOLLOWS:

· Chapel, Memorial Service, Local Church or Cemetery Service $ 690.00
· Evening Funeral Service at a Local Church after 5:00 p.m. $ 790.00
· Evening Vigil Service or Trisagion Service at a Local Church $ 790.00
· Weekend Funeral Service at Chapel, Church or Cemetery $ 790.00
· Viewing or Visitation Room 2 hours (thereafter $345.00 per hour) $ 690.00

USE OF AUTOMOTIVE EQUIPMENT

FEES ARE AS FOLLOWS:

· Use of Funeral Hearse/Coach In Alameda County $ 345.00
· Use of Delivery Vehicle for Transfer of Remains /Cremains / Flowers to Local Cemetery, Crematory or Airport in Alameda County $ 195.00
· Witness Cremation / Transfer to Crematory $ 245.00


NOTE: ALL VEHICLES ARE BASED ON A 20 MILE RADIUS FROM THE MORTUARY, ADDITIONAL DISTANCE WILL BE CHARGED AT A RATE OF $2.35 PER MILE.

DIRECT CREMATION

$ 750.00

THIS FEE INCLUDES TRANSFER FROM THE PLACE OF DEATH IN ALAMEDA COUNTY AND BASIC SERVICE OF FUNERAL DIRECTOR AND STAFF.

If you desire to arrange a Direct Cremation, you may use an unfinished wood box or an alternative container. Alternative containers encase the body and can be made of materials like fiber board or composition materials with or without outside covering. The containers this mortuary provides for cremation is on our Casket Price List. A complete list is available at the mortuary.

YOU MAY PROVIDE YOUR OWN CREMATION CONTAINER; HOWEVER, ALL SUCH MERCHANDISE MUST MEET ANY AND ALL APPLICABLE CALIFORNIA STATE OR CEMETERY REQUIREMENTS.

Direct Cremation – purchaser provides container $ 750.00

THIS FEE DOES INCLUDE:

· 24 Hour Availability / Basic Service of Funeral Director and Staff
· Obtaining all legal documents, certificates and permits
· Transfer of Remains in Alameda County

THIS FEE DOES NOT INCLUDE:

· Crematory Charges
· Witness Cremation / Transfer to Crematory $ 245.00
· Minimum Fiberboard Container with no exterior or interior
· Certified Death Certificates / Cashed Advance Items / State Sales Tax
· Additional Crematory / Transfers Charges may apply for few selected individual cases with a (High Body Mass Index-BMI) of 35 or more or over 300 lbs.
· *Any additional person needed for a transfer will be additional fee of $ 100.00
· Use of Facilities for Funeral Service or Memorial Service
· Disposition of Cremated Remains / Scattering / Burial / Mailing
· Coroner’s Processing Fee
· Refrigeration, in compliance with California State Law to require refrigeration of an non-embalmed remains. (After first 48 hours $75.00 per day.)

DIRECT BURIAL

$ 895.00

THIS FEE INCLUDES: BASIC SERVICE OF FUNERAL DIRECTOR AND STAFF, TRANSFER OF REMAINS FROM THE PLACE OF DEATH AND DELIVERY OF REMAINS TO A CEMETERY IN ALAMEDA COUNTY.

You may provide your own burial container (casket) however all such merchandise must meet any and all applicable state or cemetery requirements.

Direct Burial – purchaser provides container      $ 945.00
Direct Burial – with minimum casket                   $ 1,340.00

THIS FEE DOES INCLUDE:

· 24 Hour Availability
· Basic Service of Funeral Director and Staff
· Obtaining all legal documents, certificates and permits
· Transfer of Remains in Alameda County
· Delivery of Remains to a Cemetery in Alameda County

THIS FEE DOES NOT INCLUDE:

· Certified Death Certificates / Cashed Advanced Items / State Sales Tax
· Any Cemetery Charges / Disposition of Decedent
· Use of Facilities for Funeral Service or Memorial Service.
· Coroner’s Processing Fee
· * Any additional person is needed for a transfer will be additional fee of $ 100.00
· Additional Crematory / Transfer charges may apply for a few selected individual cases with a (High Body Mass Index – BMI) of 35 or more or over 300 lbs.

FORWARDING REMAINS TO ANOTHER FUNERAL HOME

THIS FEE INCLUDES:

· 24 Hour Availability / Basic Service of Funeral Director and Staff
· Obtaining all legal documents, certificates and permits
· Transfer of remains from place of death in Alameda County.

THIS FEE DOES NOT INCLUDE:

· Any Cash Advance Items / Death Certificates / Burial Permit
· Embalming of Remains (if necessary)
· Burial Container (Casket)
· Applicable California Sales Tax
· Transportation to Receiving Funeral Home
· Coroner’s Processing Fee
· *Any additional persons is needed for a transfer will be additional fee of $100.00

RECEIVING REMAINS FROM ANOTHER FUNERAL HOME

$ 750.00

THIS FEE INCLUDES:

· Basic Service of Funeral Director and Staff
· Transfer of remains from place of pickup/Local Airports (If needed)

THIS FEE DOES NOT INCLUDE:

· Any Cash Advance Items
· Use of Facility for Funeral Ceremony
· Use of Vehicles for Funeral Ceremony
· Burial Container (Casket)
· Applicable California State Sales Tax
· * Any additional person is needed for transfer will be additional fee of $ 100.00
